Referral form downloads
Referrals must be completed by;
a Local Medical Officer (LMO) or General Practitioner (GP),
a Nurse Practitioner specialising in a community nursing field,
a treating doctor in hospital,
a hospital discharge planner, or
a Veterans’ Home Care (VHC) Assessment Agency.
All care staff providing services have relevant clearances and qualifications, including undertaking the NDIS Worker Orientation Module, National Police Check, DCSI clearances – Aged, Vulnerable, Disabled and Child, manual handling, and CPR. All nurses are AHPRA registered at Pop-Up Community Care and have minimum mandatory standards required.
