We have Registered Nurses and Enrolled Nurses providing caring, professional quality comprehensive care for your nursing requirements.   We assist you to live in your own home longer by delivering and promoting dignity and respect.   We care for patients with both short and long term nursing requirements. No referral is required for our private nursing services. 
 
​As an advocate for your health and wellbeing, we ensure that your choices are respected and that you are provided with as much information as possible for decisions about your health and treatment.

​Working as a team and partnering with your General Practitioner, ensures you receive the very best care, without having to leave your home.
 
Our private fee for service nursing is available 24 hours a day, 7 days a week.  Our services include;

veteran home care

Veterans' Home Care (VHC) is a DVA program designed to assist eligible DVA clients who need a small amount of practical help to continue living independently in their own home. Services that our team can assist with include;
  • Domestic Assistance
  • Personal Care
  • Respite Care
  • safety-related Home and Garden Maintenance.

The VHC Program also assists carers in recognition of the vital role they play in the veteran and defence community. VHC is only available for Yorke Peninsula and Goolwa regions.
To receive VHC services, you will need to be assessed by a VHC Assessment Agency as needing home care assistance;

DVA Community nursing

We provide In-home nursing care to Veteran clients under the Dept of Veterans Affairs (DVA) Community Nursing Program. The DVA Community Nursing Program provides services for entitled persons to meet their assessed clinical and personal care needs. We provide services across all of Metropolitan Adelaide, Yorke Peninsula, Barossa Valley, Kapunda, Quorn, Goolwa, Victor Harbor and the Adelaide Hills region.

Our nursing services are delivered by Registered Nurses, Enrolled Nurses and Personal Care workers and the aim of the program is to enhance our clients independence at home and health outcomes by avoiding early admission to hospital and/or residential care.  Our services include;

  • wound management
  • personal care (including showering)
  • medication management
  • eye drop administration
  • symptom management
  • bereavement care
  • palliative care

Having Pop-Up Community Care as your provider will ensure that you are met with professionalism and respect through-out the planning and delivery of your services and our high quality standards will ensure your care is delivered with a person centred, fund appropriate, and holistic approach.

Don’t pay an exit fee! – If you wish to transfer your services, you will not be charged for this.

Don’t pay a provider fee that you won’t see! – Your provider fee will come straight back to you and put towards your own care, to cover any extra services or consumables you may require.
Our Home Care Services include but aren't limited to:
​
  • NURSING CARE: Wound care, medication management, pain management, chronic disease management and catheter care
  • PERSONAL ASSISTANCE: Assistance with showering, bathing, toileting, dressing, meal preparation and feeding
  • CONTINENCE MANAGEMENT: Assistance with continence aids/appliances including absorbent aids, commode chairs, bedpans and enemas
  • ALLIED HEALTH/THERAPY SERVICES: Physiotherapy, occupational therapy, podiatry and speech therapy
  • TRANSPORT: Assistance with transport to appointments, shopping and other excursions
  • SHOPPING: Assistance with shopping
  • DOMESTIC ASSISTANCE: House cleaning, laundry, dishes, ironing, bed making and basic repairs.
  • GARDENING: Lawn mowing, hedge trimming, pruning and general basic garden maintenance
  • MOBILITY AIDS: Assistance with purchasing aids such as walkers, canes, mobility scooters and wheelchairs
  • 24 HR IN-HOME CARE: Includes active and inactive shifts
  • RESPITE CARE: A break for your primary carer

Our Staff

Your care and safety are important to us.  All our care staff providing services go through a thorough interview and screening process and have relevant clearances and qualifications, including National Police Check, DCSI clearances – Aged, Vulnerable, Disabled, qualifications, first aid, manual handling and CPR.

All our nurses are AHPRA registered and all our Personal Care Worker staff at Pop-Up Community Care have minimum mandatory standards required including Cert III in Aged Care.

All care staff are provided with continuing education and training on a regular basis to ensure they are up to date with the latest advancements in health care so our clients receive the best quality care possible.

We provide Aged care in accordance to the new Quality Aged Care Standards 2019 and it is en-forced throughout our scope of practice, as an organization.

The New Quality Aged Care Standards 2019 are made up of eight individual standards:

  1. Consumer dignity and choice
  2. Ongoing assessment and planning with consumers
  3. Personal care and clinical care
  4. Services and supports for daily living
  5. Organisation’s service environment
  6. Feedback and complaints
  7. Human resources
  8. Organisational governance.
Can I get a Home Care Package?

If you have not already been assessed by an Aged Care Assessment Team (ACAT) for a Home Care Package, you or your nominated representative need to contact My Aged Care on 1800 200 422 or go to www.myagedcare.gov.au

The contact centre staff may then:
  • Arrange a face-to-face assessment of your needs that takes place in your own home by an assessor
  • Provide you with aged care information and details on services that may assist you.

Home Care packages are designed to keep you living in your own home longer.  Your package belongs to you, so your choice matters.

Our Client Care Liaisons treat everyone on their individual circumstances and will work together with you to tailor your package to suit your needs and help coordinate the service you require.

There are four levels of home care packages:

  • Level 1 – Basic care needs
  • Level 2 – Low-Level care needs
  • Level 3 – Intermediate care needs
  • Level 4 – High-Level care needs

Pop-Up Community Care Nurses are passionate about the care they deliver and uphold the highest standard of professionalism and care for all our NDIS participants.
Our team includes Registered Nurses and Enrolled Nurses. We are supportive of all personal beliefs, values, and preferences. We treat each participant on their individual circumstances and our caring staff are always supportive.

We comply with all relevant NDIS legislation, practice standards, code of conduct and rules which regulate the provision of our services and we hold all registrations, permits and licenses' that are required for us to lawfully perform our obligations.
As a Registered NDIS Provider for Community Nursing Care we comply with the NDIS Practice Standards, Including:
  • Rights and responsibility for participants
  • Governance and operational management
  • The provision of supports
  • The support provision environment
  • High intensity daily personal activities

Pop-Up Community Care has recently in April 2020 undergone a Stage 2 NDIS certification audit and proud to have been found to be compliant in all areas.

Pop-Up Community Care can invoice according to which type of plan management you have. The three types of plan management through NDIA are:
  • Agency Managed – NDIS provide payment direct to Pop-Up Community Care
  • Plan Managed – A third party provider is responsible for claiming from your NDIS funding such as a Plan Management Agency. Pop-Up Community Care will then forward invoices on to this agency for payment of services we provide to you.
  • Self-Managed – You are responsible for all payment of invoices. You will be able to claim back these funds from NDIA direct. Pop-Up Community Care will invoice you on a fortnightly basis and you will have seven days to make payment.

  • Registered Nurse Assessments
  • Overnight Nursing Care
  • Wound Drain Management/Removal (Open/Closed/Suction)
  • PEG Feeding
  • IV Antibiotic Administration
  • Cannulation

All care staff providing services have relevant clearances and qualifications, including undertaking the NDIS Worker Orientation Module, National Police Check, DCSI clearances – Aged, Vulnerable, Disabled and Child, manual handling, and CPR. All nurses are AHPRA registered at Pop-Up Community Care and have minimum mandatory standards required.
